5.Medical care・birth・annuity

5-1 Citizen health Insurance

[ Citizen health Insurance ]
Citizen health Insurance is a joint medical system involving the premium of the Insurance policy holder and national burden. It is self-managed by the city, district and village. Foreigner who plans to stay in Japan for over one year and has registerd while has not jioned health Insurance or other Insurances should join in this kind of Insurance.

[ Medical charges burden of citizen health Insurance ]
Those who has joined citizen health Insurance can accept medical examination at hospital by providing the health Insurance certificate and pay 30% of the medical charges.

[ Medical charges not included in citizen health Insurance ]
Charges of normal pregnancy, birth–giving, bed-difference, operation or treatment for beautification purpose, health examination, vaccination etc are not included in citizen health Insurance.

[ Other items ]

• Expensive hospitalization charges
  If one stays in a hospital for one month, if the medical charges exceeds the settled coverage, one may xubmit an application and get some margin.
• Infant birth giving – lump-sum payment
• If those who have joined citizen health Insurance give birth, a lump-sum payment may be obtained upon application.
• Funeral expenses
  If those who have joined citizen health Insurance die and a funeral will be held, a sum of funeral payment may be obtained upon application.
• Unsuitable conditions:
  Charges of normal pregnancy, birth–giving, bed-difference, operation or treatment for beautification purpose, health examination, vaccination etc are not included in citizen health Insurance.

[ Join in ]
In the folowing cases, one should handle the joining procedures within 14 days

1. Moving into this city from other places,please handle the joining procedures with foreigner registration certificate
2. Resign, retire or terminate the health Insurance at the working unit, please handle the joining procedures with foreigner registration certificate and social Insurance qualification loss certificate.
3. After giving birth, please handle the joining procedures with foreigner registration certificate and citizen health Insurance certificate of the householder or one of the parents.
4. Just arrive in Japan, Please take you passport.

After joining precedure is finished, please give the citizen health Insurance certificate to the householder and the Insurance payment note should be issued.

[ Withdrawal ]
In following cases, please handle the withdrawal procedures and return the citizen health Insurance certificate to the city hall within 14 days.

1. Move out of this city, please hande the joining procedures of citizen health Insurance in the new city or place.
2. Start to wok in a new company and join the health Insurance of the company. Pleae handle the withdrawal procedures at the window of the city hall with the new health Insurance certificate and the citizen health Insurance certificate.
3. The insured dies
   ( the insured goes abroad.

[ Other procedures ]
In following cases, please declare to the city hall with foreigner registration certificate and citizen health Insurance certificate within 14 days.

1. Move to a new address within the city.
2. Householder’s name changes or the family members changes.
3. Babies are born.

5-2 Long life ( seniority ) medical system

[ What is long life medical system ]
It is a medical system for those over 75 years old and those over 65 years old who have been appraised as a certain deformity in Fugaoka county.

[ Treatment in long life medical system ]
Those who join the medical system can take medical care with Insurance certificate and pay 10% or 30% of the medical charges in accordance with their income condition.

• Unsuitable cases
  Long life medical system doesn’t cover health examination, vaccination, bed difference at hospital etc.

[ Other ]

• High hospitalization charges
  One may submit an application to get back the margin exceeding the limit if he burdens high medical charges within on e month.
• Funeral charges
  If the insured dies and a funeral will be held, the funeral holder can submit an application and get the funeral expenses.

[ Target ]
Those who are over 75 years old ( those who are over 65 years old and appraised as disformity ) Due to the reaons of staying qualification and staying period, may have a certain limit. Foreigner registration certificate may be required to present when apply for joining the medical system.

[ Other procedures ]
In the following cases, please return the long life medical system Insurance certificate to the city hall.

1. Move out of the city.( Please apply for the long life medical system Insurance certificate to the new city or villages)
2. Death

5-3 Various kinds of health examination

Those foreigners aged 35-39 years old who have registered in this city can enjoy health examination, blood exammination etc with a relatively low charges.
The examination period is from June to November while the examination places are contracted medical organizations with the city hall ( There are 190 such kind of medical organizations in the city.)
Medical examinations are provided for citizens over 40 years old who suffer stomach cance, lung cancer, intestine cancer and breast cancer ( for females of even number ages), females over 20 years old who suffer uterus cancer, males over 50 years old who suffer prostate cancer etc. In addition. Females can have a bone examination every five years between 25 and 70 years old. Health daily code is required at examination.

5-4 8020 teeth examination ( teeth disease examination)

Examination period is from June to March the next year. The places are contracted dental organizations ( dental hospital and clinic) Examination target people are those who are 40, 50, 60, and 70 years old.

5-5 disease or injury

[ Go to the doctor ]

• Comprehensive hospital
  Appointment is rerquired at dental hospital, the patients can get treatment in accordance with the order of registration at other hospitals. First, show your health Insurance certificate at the registration window and explain your problem and which department you wish to take examination, then follow the instruction to relevant department. Pay the medical charges at the payment counter after being examined, you may get the medicines girectly at the hospital or you may take the inscription to buy medicine at the drug store.
  Don’t forget to show your Insurance certificate , or you have to pay full amount of the expenses
• For emergency, please dial 119 for ambulance.
  In case of disease or injury on weekends
• If one suffer disease or injury on weekends, please use weekend home doctors. Home doctors adopt duty system on weekend. 5-6 Mental care consultation

5-7 Consultation and examination of AIDS. Sex infectious disease

5-8 Pregnancy and birth giving

[ Pregnancy ]

• Pregnant and plan to give birth in Japan, please submit a birth-giving application to health promotion department or city people center and get mother & infant health brochure to take note of the growing and vaccination of the infant. In addition, the pregnant woman can also get supplement tickets of pregnant woman health examination ( five times ) and accept guidance from birth assistant and take lectures about pregnant and baby care knowledge in mother classroom.
• Birth assistant guidance and mother classroom lectures will provide necessary assistance to borht giving and infant care.
• Provide health consultation about non-pregnancy, climacterium for women.
• Provide medical subsidy ( twice a year ) to couples who implemented special non-pregnancy treatment ( artificial fertilization) ,150,000 yuan at the first time and 100,000yuan at the second time.

[ Birth giving ]
Birth application should be submitted to city people department 14 days before the birth giving date. Mother and infant health brochure can be obtained even if the infant was born abroad.

• Birth assistants can provide guidance for the first birth or special birth giving mothers after the birth giving. And they can provide visit and consultation service for those infant whose weight is less than 2500g.
• Babies can take examination 4 months, 10 months, 18 months and 36 months after it was born, the medical organization will send a letter to remind about this.
• Services such as mother milk breeding stop classroom, infant safety classroom and infant growing consultation are provided.
• Normal prevention and vaccination are provided for infants. Some vaccination will be conducted in group while the date and places of vaccination will be notified on newspapers and website, please read the sheets issued by health care department and city people center for the date and places of special vaccination.

5-9 Citizen annuity

[ Joining and annuity payment ]
Foreigners aged 20-60 years old who have registered while not joined annuity in the company should join citizen annuity and pay annuity Insurance fee every month. Seniorty basic annuity, disabled basic annuity and other annuities.

[ Qualification loss and lump sum payment for annuity withdrawal ]
In condition that one has not a fixed residence in Japan, he should submit qualification loss declaration for citizen annuity. Those who have paid annuity premium for six months and go abroad before withdrawing the annuity can apply within two years agter going abroad to get the lump sum payment. The payment varies in accordance with the annuity Insurance continuous payment period.
